This next bit of information can also be found on the order forms at http://www.nickspradlin.com
"If you are going to be paying a wage to yourself or other employees, state law requires you to have an Unemployment Tax ID so that you may comply with the state unemployment withholding's. You will use the Unemployment Tax ID to mail in your quarterly unemployment withholding's due to the state."
As the above information states, you must have this unemployment tax account number to pay yourself, or your employees, a wage. You can acquire this number by having an attorney acquire it for you or you can acquire it yourself at http://www.myflorida.com. This order form will span across several pages and take quite some time, but you can indeed do it yourself. Make sure you have already set up a company for the unemployment tax account number to belong to. If you haven't already set up a company, you can do this at The Law Offices of Nick Spradlin, PLLC. This is where I set my company up and it was no hassle whatsoever. You can visit their site at the link located at the top of this block. www.nickspradlin.com
